The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks a Housing Intake Coordinator to complete housing packets for clients receiving vouchers through the coordinated entry and assessment process (OneHome) or through other CCH Programs. This position will assist with locating bridge housing and gathering documentation necessary to issue housing vouchers. Services are provided throughout the Denver Metro Area, including Jefferson, Arapahoe, Adams, Boulder, and Denver Counties and in close collaboration with several area Community Mental Health Centers. This position is located at 2100 Broadway, Denver, Colorado 80205 and reports to the Program Manager.

Essential Job Functions

  • Identifies affordable housing units, develops and maintains positive relationships with local property owners and landlords.
  • Assists in linking clients to necessary community support services including behavioral health, medical, dental, eye, pharmacy and any other medically necessary resources or community support.
  • Attends and participates in OneHome meetings in the community
  • Outreaches clients in community to ensure documentation is gathered in timely manner.
  • Completes housing packet to include:
  • Vital Documents (Birth Certificate, CO ID, SS Card)
  • Disability verification
  • Documentation of Chronic/Literal Homelessness
  • Completes initial HQS inspection.
  • In coordination with Program Manager, provides training on housing retention and landlord recruitment to community agencies and within CCH as requested.
  • Guides clients through process of housing orientation and lease up in coordination with agency responsible for issuing voucher.
  • Meets with clients to review scattered site rental options.
  • Ensures smooth transition of client to ongoing clinical and/or case management services, including linking clients with ACT teams, or other appropriate levels of care.
  • Serves as liaison between landlord/property owners, clients, and agency providing ongoing supportive services.
  • Provides relocation assistance to clients as needed (identify new unit, complete all necessary forms for relocation).
  • Maintains housing and case management databases for housing programs (IDIS, File Maker, HMIS).
  • Collaborates with other Coalition programs and outside service providers to ensure a continuum of care for program participants.
  • Consults regularly with supervisor regarding training needs, operational issues surrounding intakes, as well as any other issues that may arise concerning housing and supportive services programs.
  • Networks with community agencies, especially homeless service and employment service providers, for the purpose of information, coordination, and/or development of services. This may include attending community meetings, committee work, and/or presentations to interested agencies.

Qualifications Summary

  • Bachelor’s degree in a related field required. Relevant experience may substitute for education.
